Contact Form
Display a customizable contact form that allows customers to send messages directly from your storefront.
The Contact Form section provides a clean and accessible form for customers to get in touch with your store. It includes required fields for full name, email address, and a comment message, with optional fields for phone number and order number. The form supports Ajax submission with inline success and error feedback, and an optional response time note displayed below the form.

How to Edit the Contact Form Section
- From your Shopify admin, go to Online Store → Themes.
- Click Customize on the active theme.
- Select the Contact Form section from the left sidebar.
- Adjust the settings according to your preferences.
- Click Save to apply your changes.
Section Settings
General
- Color Scheme: Select a color scheme applied to the section.
- Heading: Enter the page title displayed above the contact form.
- Short Description: Add an optional subtitle or short description displayed below the heading.
- Padding Top: Controls the top padding of the section.
- Padding Bottom: Controls the bottom padding of the section.
Form Fields
- Show Phone Number Field: Enable or disable the optional phone number input field.
- Phone Placeholder: Customize the placeholder text for the phone number field. Only available when Show Phone Number Field is enabled.
- Show Order Number Field: Enable or disable the optional order number input field.
- Order Number Label: Customize the label displayed above the order number field. Only available when Show Order Number Field is enabled.
- Order Number Placeholder: Customize the placeholder text for the order number field. Only available when Show Order Number Field is enabled.
- Submit Button Label: Customize the text displayed on the form submit button. Default: Send Message.
- Show Response Time: Enable or disable the response time note displayed below the form.
- Response Time Text: Customize the response time message. Default: Average response time: within 24 hours.

Form Fields
The contact form includes the following fields.
Always Visible
- Full Name: Required text field for the customer's full name.
- Email Address: Required email field for the customer's email address.
- Comment: Required textarea for the customer's message.
Optional
- Phone Number: Optional telephone input field. Enable via Show Phone Number Field setting.
- Order Number: Optional text field for referencing a specific order. Enable via Show Order Number Field setting.
Best Practices
Do
- Keep the heading and description concise and welcoming.
- Enable the phone number field when your support process involves follow-up calls.
- Enable the order number field when handling order-related inquiries.
- Set a realistic and accurate response time note to manage customer expectations.
- Customize the submit button label to match the tone of your store.
Don't
- Enable optional fields that are not relevant to your support workflow.
- Leave the response time note enabled without updating it to reflect your actual response time.
- Use a vague or generic heading that does not communicate the purpose of the form.
- Leave the heading and description blank as this reduces context for customers.
- Use the contact form as the only support channel without providing alternative contact options elsewhere on the page.
